### What causes LLM hallucination?

LLMs hallucinate because they are trained to predict the most statistically likely next token — not the most factually accurate one. They have no internal fact-checking mechanism. ### How common is hallucination in enterprise LLM deployments?

### Does RAG eliminate LLM hallucination?

RAG is the most effective single technique available, but it must be combined with other controls.
